About Our Office

Our Conyers location is open five days a week.  Our typical office hours are listed but are subject to change due to the physician being called for an emergency.  We answer our phones beginning at 8:30 am to 4:30 pm Monday through Friday.  The phones are forwarded to an answering service after 4:30 pm and on the weekend.

Conyers hours:
8:30 am to 4:30 pm Monday through Friday

Lithonia hours:
8:30 am to 4:30 pm Tuesday, Thursday and Friday

Appointment Policy
The staff of New Beginnings work extremely hard to accommodate their patients with their appointment(s).  We ask that you arrive 15 minutes prior to your scheduled appointment time to allow for you to complete any required forms or to update your insurance or contact information.  If you are more than 30 minutes late for your appointment, you may be asked to reschedule. A $25 “no show” policy has been implemented as a result of those patients that routinely miss appointments which may result in being discharged from the practice.

After hours Emergency
Emergency calls must be made through our main phone number 678-413-0858.  Your call will be forwarded to our answering service who will page the provider on call.  A physician is on call 24 hours a day, 7 days a week.

Prescription Refills
Requests for prescription refills will be processed Monday through Friday during the hours of 8:30 am to 4:30 pm only.  If you have been in our office within the past year and are in need of a refill to get you through to your next appointment, call your pharmacy and have them fax a refill authorization to our office.  You do not need to call our office.  Your request will be reviewed and processed within 72 hours.

Scheduling Annual Exams
Routine annual exams (Pap Smear) are usually scheduled following your recent appointment.  We limit the number of annual exams each day to accommodate emergencies and our obstetrical patients.  We
appreciate your patience when scheduling and suggest that you schedule your annual at least 90 days in advance if you did not schedule after your recent appointment.

Test Results
It is your responsibility to provide our office with the most recent contact information.  The tests ordered by your provider are monitored by our in-office staff.  You will be notified by phone or mail of any abnormal result.  It is our policy not to leave test results on a patient’s voice mail. Please allow at least 7 working days for all test results to return to our office.  Currently, you may call our office after the 7 day time period to verify normal or abnormal results.

Completion of Forms
New Beginnings understands that our patients may need forms to be completed by their provider (disability, FMLA, work release, etc.).  We try to complete these forms as quickly as possible.  We require that the patient section be completed BEFORE giving us the forms.  There is a $10 fee for each form to be processed that must be paid prior to completing the form(s).

Copying of Medical Records
We will provide you with a copy of your medical record after we receive a valid signed request.  There is a charge for copying your records which complies with the State of Georgia fee schedule.  We will inform you of these charges.  Please allow 5 business days to process your copy request.

Patient Confidentiality
Patient confidentiality laws limit to whom we can release information.  These laws are to protect you and your confidential health information.  Thank you in advance for your understanding and for any inconvenience this may cause you.
